If you decide to move forward, you will complete a secure medical intake through the partnered-provider portal. You’ll provide the health information needed for a licensed provider to evaluate whether a treatment option may be appropriate for you.
3. Provider Evaluation & Treatment
Your information is reviewed by a licensed medical provider. The provider determines whether treatment is medically appropriate and, when appropriate, handles prescribing and treatment management directly through the portal.
4. Convenient Virtual Care
The medical portion of these services is handled virtually through our partnered medical providers, allowing you to access care privately and conveniently without an in-person appointment at Jacobs Lab Services.
Self-Pay Services
Insurance is not accepted for these services. All treatment options are self-pay. Pricing, payment, medical intake, provider evaluation, and prescription services are completed through our partnered-provider platform.
Prescription treatments require evaluation and approval by a licensed medical provider. Treatment is based on individual medical eligibility and is not guaranteed.
